An affidavit is a sworn statement. The person giving the sworn statement is known as the affiant. The affiant makes his sworn statement in front of the notary. The notary then signs the affidavit declaring that the affiant in the person stated within, and that the sworn statement was created on a certain date, in a certain parish. The affidavit may be admissible in a court of law, with a government agency, and may be held by private parties.

I will need your full legal name and possibly other identifying information such as your date of birth, address, social security number, etc. I then need you to give me your statement either verbally or in writing, I will then create the actual affidavit with your information. After you read it and agree to the content, you will sign in in front of me, I will sign it, and then I will place my notary seal on it.
